Corporation – Business name contains “company,” “corporation,” “incorporated,” or an abbreviation
- EIN (Employer Identification Number)
- Articles of Incorporation as filed with and stamped by the appropriate state official

Partnership - (General, Limited) – An unincorporated business owned by 2 or more people
- EIN (Employer Identification Number)
- Partnership Agreement
- Valid business license
- Certificate of Limited Partnership as filed with and stamped by the appropriate state official or other government issued license verifying the business name

Sole Proprietorship – An unincorporated business owned by an individual
- Employee Identification Number (EIN) or a Social Security Number (SSN)
- Valid business or occupational license
- Fictitious Name Certificate or Certificate of Trade Name (if applicable) or Assumed Name Certificate, as applicable in state law

Limited Liability Company - Business name contains "LLC"

- Valid Business License
- Articles of Organization
- Assumed Name Certificate, applicable in state law

Unincorporated Organization or Association – Nonprofit business
On the organization's letter head, include:
- Minutes from the last business meeting
- List of authorized signers, officers, and trustees
